SUPERVISOR'S EVALUATION

 

Supervisors will be sent an evaluation form and return envelope.  It is the student's responsibility to see that the supervisor completes the evaluation in a timely manner.  Evaluations must be mailed and received, or given directly to the instructor during a site visit, before a grade will be issued.  Evaluations should be mailed to insure a timely receipt.

Students are encouraged talk with their supervisors about their performance.  Having an open discussion regarding strengths and areas needing  improvement can be very beneficial in helping the student to make adjustments to improve their performance.

Evaluation Criteria

Employers will be asked to evaluate interns on the following criteria:

  • Attitude and application to work -- interest and enthusiasm

  • Dependability

  • Knowledge of field

  • Potential as a professional

  • Ability to learn

  • Initiative

  • Ability to organize work and self

  • Responses to supervisors and administrators

  • Quality of work

  • Judgment

  • Relations with peers

  • Leadership qualities

  • Attendance

  • Punctuality

Employers will also be asked to give an overall evaluation and to identify the intern's outstanding professional qualities and areas for improvement.  Employers have the option of sharing their evaluation with the intern.
